Son's 12th Annual Spaghetti & Meatball Fundraiser for Erie County Cancer Services will be held on Sunday, April 12th, 2015 from Noon - 7:00 pm. The Benefit will be held at the Post with Tickets costing $8.00. Drive-thru Carryout available. Raffles, 50/50 & 1st place drawing worth $1,000.00. Widow: Vet's death certificate: Notarized Discharge papers: Copy of Marriage License (if married): Divorce papers or death certificate for all prior marriages of either the Vet or spouse/widow(er); Social Security Number and award letter for each household family member; Income data for each household family member: pre. year; Interest statements or account books from Banks, etc. for pre. year; Amt. of Company or union pension benefits before deductions: pre. year; Unemployment benefits (including 1st and last checks) for pre. year' Sick pay or disability pay, Workman's Comp. (1st & Last Checks) pre. year: Life Insurance proceeds (If applicable); Employment Earnings (Pre. year); Net business or rental income (pre. year); SSI benefits (pre. year); FIA (Dept. Of Soc. Services) assistance (Includes ADC and Gen'l Assistance) for pre. year; Any other income source (e.g. Land Contract, Savings Bonds, Stocks, Money Certs., IRA's, 401K's, Royalties, Trusts, etc.) for pre. year; Current value of savings and/or checking accounts, real estate (excluding home), or non essential personal property; Out Of Pocket medical expenses for pre. year for Vet and/or Spouse: (health/supplemental insurance premium, nursing home/home healthcare expenses and prescriptions: if in a nursing home, has the VA Form 21-0779 been completed (and/or attendant affidavit)?; and if required the Aid and Assistance to perform life's daily necessities, has the VA Form 21-2680 been completed by your doctor? You may not need all of the above documents to apply but you do need all the above information to apply. Claims based on non-service connected disability or widow's person generally require active duty during a recognized wartime period.
